Please note that this cost and process is for sending out a replacement card and is not equivalent achieving the initial qualification. Please refer to CPCWHS1001–Work Safely in the Construction Industry (White Card) QLD, for enrolment details if you have never received this certification before. The older Queensland Blue Cards are no longer accepted and cannot be replaced.
The replacement White Card process is as follows:
1. Read our introduction to make sure you qualify for a replacement card
2. Provide us with some personal details & delivery address
3. Provide us with your signature—this is done online (we need it to put on your new card)
4. Complete the Statutory Declaration, outlining the circumstances in which your card was lost, stolen or destroyed. This declaration must be witnessed by an authorised person.
